A Controller of Examinations at a mid-sized university signs off on three hundred question papers per semester. Each paper is supposed to cover specific course outcomes, maintain a defined difficulty balance, distribute questions across Bloom’s cognitive levels, and follow the institutional blueprint. But can the CoE actually verify any of this before signing? In most institutions, the answer is no. The paper arrived as a Word file. The blueprint was a suggestion, not a constraint. The only evidence that the paper is sound is the setter’s professional judgment — undocumented, unauditable, and unrepeatable. Without a governance system, the CoE is signing blind.

They email the paper to a reviewer, who reads it and sends back comments. A revised version is emailed again. At some point, someone declares it final. The paper is printed, stored, and delivered to the examination hall. At no stage does a system enforce that the paper covers all mapped COs, that cognitive-level distribution matches the blueprint, or that the paper is equivalent in difficulty to the paper set for a parallel cohort.

Question banks, where they exist, are unmanaged repositories. Faculty contribute questions over years, but there is no systematic review of bank health. Topics may be overrepresented or completely absent. Questions may be tagged to COs incorrectly — or not tagged at all. Duplicates accumulate. Questions written for an older syllabus persist alongside current ones. No one audits the bank because no one can: the data is not structured in a way that supports analysis.

Version control is nonexistent. When a paper goes through multiple drafts, earlier versions are not systematically preserved. If a dispute arises after the exam — a leaked question, an out-of-syllabus item, a mismarked answer key — there is no authoritative record of who set the question, who reviewed it, who approved it, and when each action occurred. The audit trail simply does not exist.

Assessment parity across cohorts is aspirational at best. When multiple sections or shifts require equivalent papers, parity depends on the paper setter’s judgment. There is no computational guarantee that Paper A and Paper B cover the same COs in the same proportions, at comparable difficulty levels, using non-overlapping questions. Any variance is invisible until it surfaces as a student grievance — or an accreditation finding.

Security gaps compound the governance problem. Papers stored as Word files on personal drives, emailed between faculty, printed in-house, and transported by hand create multiple points of vulnerability. Paper leak incidents are not always the result of malicious intent; sometimes they are simply the consequence of ungoverned handling.

Why It Matters Now

Each role’s action should be logged with identity and timestamp. This creates the audit trail that accreditation bodies expect — and that institutions need for their own defensibility.
